Participants or Guardians of participants in National Elite Hockey programs of any type must read and acknowledge agreement of the following terms and conditions. You will be restricted at checkout without acknowledging your agreement.

Insurance Waiver

The risk of injury from the activities involved in playing Ice Hockey is significant, including the potential for permanent paralysis and death. While particular rules, equipment and personal discipline may reduce this risk; the risk of serious injury does exist, and, as a parent or for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS National Elite Hockey LLC/Clifford John Loya III their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors and advertisers; from any claims arising out of my or my child’s participation in these activities.

COVID-19 Waiver

The Novel Coronavirus, Covid-19, has been declared a worldwide pandemic by the World Health Organization. Covid-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state and local governments and federal and state health agencies recommend social distancing and have in many locations prohibited the congregation of groups of people.

National Elite Hockey Development, LLC (NEH) has put in place preventative measures and worked with the facilities it operates in, to reduce the spread of Covid-19; however, NEH cannot guarantee that you or your child(ren) will not become infected with Covid-19. Further, attending sessions with NEH could increase your risk and your child(ren)’s risk of contracting Covid-19. By signing this agreement, I acknowledge the contagious nature of Covid-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by Covid-19 by attending sessions conducted by NEH and that such exposure or infection may result in personal injury, illness, permanent disability and death. I understand that the risk of becoming exposed to or infected by Covid-19 at a NEH event, may result from the actions, omissions, or negligence of myself and others, including, but not limited to, NEH owners, officers, employees, sub-contractors, agents, volunteers, facility management & staff, program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connections with my child(ren)’s attendance in a NEH event or any participation in NEH programming (“claims”). On my behalf, and on the behalf of my child(ren), I hearby release, covenant not to sue, discharge, and hold harmless NEH, its owners, officers, employees, sub-contractors, agents, representatives, volunteers, facility management & staff of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of NEH, its owners, officers, employees, sub-contractors, agents, representatives, volunteers, facility management & staff, whether a Covid-19 infections occurs before, during, or after participation in any NEH program.

Media Policy

I hereby grant National Elite Hockey Development, LLC the right to use photographs, video images and/or other media of my child for publicity, advertising and/or other commercial purposes. I understand that this event and future National Elite Hockey Development, LLC, events may be photographed, videotaped or otherwise recorded. I agree to let National Elite Hockey Development, LLC, use my name, child’s name, photo likeness of myself and my child and demographic information free of charge in any manner and for any purpose in any media now known or here after created. National Elite Hockey Development, LLC, will not sell any information or likeness to a third party.

Refund Policy

  • All cancellation requests must be received in writing to nelitehockey@gmail.com. Failure to attend a program without notifying National Elite Hockey Development, LLC, will result in no credit to a future program. Please allow 30 days for requests to be reviewed. Illness and injury are the only valid reasons for cancellation. A credit for 50% of the unused portion will be issued. This includes if a player gets ill or injured during a program. A doctor’s note is required to receive a credit. Credits will not be issued for any other reason. Credits will expire one year from the date they were issued.
  • National Elite Hockey Development, LLC cannot provide refunds for cancellations due to unforeseen acts of God, including facility damage/closing, complications or weather related rink/facility cancellations, acts of government, acts of war, epidemic/pandemic, or any other public health emergency. These are highly unlikely to occur, but in the event it does, we will issue credit vouchers for the prorated amount of the remainder of the effected program.
  • If National Elite Hockey Development, LLC cancels a program for any other reason not specified above, we will offer a refund.
  • National Elite Hockey Development, LLC reserves the right to cancel or change the program, dates, times and locations.
  • National Elite Hockey Development, LLC has zero tolerance policy with respect to uncontrollable behavior, bullying, hazing, alcohol, tobacco, drugs and controlled substances, weapons of any kind, or any kind of activity we deem to be illegal or not in conformance with the National Elite Hockey Development, LLC standard. If a participant is expelled for any of the reasons above, no refund or credit will be offered.

Refund Policy

  • Families must make a $250 team commitment fee to reserve your player’s roster spot. This will be done via online registration with a credit or debit card. A player’s spot is not confirmed until this commitment fee is paid.
  • The commitment fee is nonrefundable except for an injury or illness that will cause the player to miss the entire spring season. In this case, a credit will be issued for $250. A doctor’s note is required to receive the credit. Credits will expire one year after the date they were issued.
  • The remaining balance is due on March 1 of the calendar year. If the balance is not paid by March 1 of the calendar year, National Elite Hockey reserves the right to find another player for that team. National Elite Hockey reserves the right to keep the $250 non-refundable deposit in these circumstances.
  • If a child cannot play because of injury or illness after March 1 of the calendar year, a credit for $250 will be issued for programs in that calendar year. A doctor’s note is required to receive the credit. Credits will expire one year after the date they were issued.
  • National Elite Hockey Development, LLC cannot provide refunds for cancellations due to unforeseen acts of God, including facility damage/closing complications or weather related rink/facility cancellations, acts of government, acts of war, epidemic/pandemic, or any other public health emergency. These are highly unlikely to occur, but in the event it does, we will issue credit vouchers for the prorated amount of the remainder of the affected team.
  • If a tournament is canceled for any reason, National Elite Hockey Development, LLC will issue a credit for the tournament portion of the spring team fee. This portion amount would be the total team tourament fee divided by the number of players on the team. This credit will expire one year after the date they were issued.
  • If National Elite Hockey Development, LLC cancels a team or practice for any other reason not specified above, we will offer a refund.
  • National Elite Hockey Development, LLC reserves the right to cancel or change the program, dates, times and locations.
